This Terms and Conditions Agreement discloses the Terms and Conditions of www.resetpediatrics.com (hereinafter: the “Website”), owned by RESET Pediatric Lifestyle Medicine, a Clinic in the Province of Ontario (hereinafter: the “Clinic”). By accessing or using our Website, you hereby agree to be bound by the Terms and Conditions incorporated herein and to our Disclaimers and Privacy Policy. If you do not expressly agree to all of the Terms and Conditions outlined herein, then please do not access or use our Website.

The material appearing on this Website is provided as either information about the Clinic’s self-promotion, blog, and/or services. The owner of this Website, and its directors, agents, employees, and affiliates assume no responsibility or liability for any consequence resulting directly or indirectly from any action or inaction you take based on the information found on, or material linked to, on this Website.

Your access to and use of this Website signifies your acceptance and agreement of the Terms and Conditions.

2.1 The following terminology applies to these Terms and Conditions, Privacy Policy and Disclaimer Notice: “Patient”, “User” “You” and “Your” refers to you, the person using this Website. “The Clinic”, “Ourselves”, “We”, “Our” and “Us”, refers to the Clinic. “Party”, “Parties”, or “Us”, refers to both the Patient and the Clinic. Any use of the above terminology or other words in the singular, plural, capitalization, and or he/she/they, are taken as interchangeable and therefore referring to the same.

3.1 Any and all information by or on this Website is provided for informational or promotional purposes only and is not to be relied upon as professional opinion, or in lieu of consultation with a Medical Professional or recognized professional. Such information extends to all digital content on this Website, including but not exhaustive of, email, blog(s), podcasts, events, any and all social media utilized, webinars and other content, whether or not they are available for purchase, as resources, education and/or informational use only.  All aforementioned content does not constitute medical or professional advice and shall not be guaranteed to be accurate, complete, reliable, current or error-free. You should not rely on the information provided on this website as a substitute for professional medical advice or treatment. The Clinic does not endorse or recommend any specific treatments, digital products or products/services mentioned on this website.

  1.  PRACTICE POLICIES 

4.1 For all prospective and existing patients, the following general practice policies apply: 

  1. All patients, and parents of such patients, are fully responsible for accurate and up to date health card information. A valid Health Card must be present for each visit. You understand that failure to bring a valid Health Card, or inaccurate information, may result in your claim to OHIP being rejected and subject to an administrative fee plus cost of visit. 
  2. If the patient does not have OHIP coverage, but has private insurance, You will be billed for the service and provided a receipt. 
  3. If your baby is a newborn, the OHIP number from the hospital is temporary. You must register with OHIP as soon as possible to obtain a valid permanent OHIP card. 
  4. Our Clinic may refer patients to other healthcare providers or specialists for further evaluation or treatment. Referrals are based on medical necessity and clinical judgment. It is the responsibility of the patient or their legal guardian to follow up on referrals and make appointments with the referred providers.
  5. All prescribed medications should be administered as directed for best results. Please notify the Clinic if you or your child exhibit a reaction to a medication or suspect there has been an error in the prescription.

  1. APPOINTMENT AND COMMUNICATION

5.1 All visits and appointments are by scheduled appointment only. The Clinic strives to provide timely and efficient care, and we appreciate your cooperation in arriving on time for your scheduled appointment. To book an appointment please email info@resetpediatrics.com. For any cancellations or rescheduling, we request you provide the Clinic at least twenty-four [24] hours notice.

5.2 Our clinic strives to provide effective communication with our patients and their families. We encourage open and honest communication and encourage patients and families to ask questions, voice concerns, and actively participate in their healthcare decisions. We communicate with patients primarily through phone calls, email, or secure online portals, and we strive to respond to messages in a timely manner. However, please note that communication through electronic means may not be completely secure or private.

  1. COPYRIGHT 

6.1 All materials created by the Clinic on the Website are protected by copyright laws as original works. The absence of a registered copyright symbol does not mean that such materials are not protected as belonging to the Clinic.

  1. LICENSE OF INTELLECTUAL PROPERTY 

7.1 Unless otherwise stated, the Clinic and/or its licensors own the property rights for all material on the Website, any digital products or products/services sold or downloaded through the Website, as well as any and all materials on the Clinic’s social media platforms, including, but not limited to all social media accounts outlined herein. All intellectual property rights are reserved. If we have materials on the Website that you can download, permission is granted to download copies of said materials for personal, non-commercial transitory viewing only.

7.2 This is the grant of a license, not a transfer of title. Under this License the User may access the Website for personal use, but the User may not:

Upon terminating your viewing of these materials or upon termination of this license, you must destroy any downloaded materials in your possession whether in electronic or printed format.

10.1 The Terms and Conditions of this Website extend to the use of social media platforms as outlined herein and any and all reviews or comments regarding your use of the services, program or information from, on or through the Website. The Clinic requests you follow and adhere to the following guidelines:

(i) The Clinic reserves the right to remove, block, and/or delete any comments that may be construed as bullying, name-calling, foul language, or contrary to the Website's intended conversation of positivity, education and encouragement.

(ii) By using any and all social media platforms, you verify that all information submitted is accurate and factual. Negative comments and/or complaints posted by you may be construed as claims about the Clinic and may be subject to legal claims.

You agree to be bound by all agreements and abide by the contents therein. All other digital products, physical products or merchandise, sold on or through this Website will be subject to the additional shipping and handling terms and conditions or a Terms of Purchase.

  1. REFUNDS

14.1 There will be no refunds of any kind for any and all digital products or products/services offered by the Clinic, sold on or through the Website. All sales of this type are complete and final. By purchasing any and all digital products or products/services on this Website, the Patient accepts without dispute the terms herein and waives any and all claims in connection with the refund policies herein.

15.1 The Clinic respects the rights and dignity of our patients and their families. Patients have the right to receive respectful and non-discriminatory care, to participate in their healthcare decisions, to have access to their medical records, and to voice complaints or concerns. Patients are also responsible for providing accurate and complete information to the Clinic, following the treatment plan provided, and respecting the rights and privacy of other patients of the Clinic and Clinic staff.
